Submission of Application Form


Application must be submitted on the prescribed application form. Before submitting an application, candidate must ensure that they have complied with the instructions given with the application form. Please make sure that copies of following documents are attached with the application, incomplete application will not be processed:

Shalamar Medical & Dental College reserves the right to revoke admission and registration if an application form is discovered to be inaccurate or incomplete, or if supporting documents are discovered to be fraudulent.

Candidates applying with downloaded application form should attach demand draft / pay order of Rs. 4,000/- in favor of “Shalamar Medical & Dental College Lahore”. No personal cheque is acceptable. Please do not send cash with postal requests. Applications complete in all respects can be sent to the Student Affairs Department not later than the dates mentioned in the schedule of admission published in the newspaper / website.

The candidate must clearly mention only one category Pakistani / Foreign / overseas to compete. In case of selecting more than one category, the candidate will be required to submit separate form for each category.
